Capital Health Medical Group is made up of more than 250 physicians and other providers who offer primary and specialty care, as well as hospital-based services, to patients throughout the region.Position OverviewChecks phone messages and returns calls as needed. Maintains residents' privacy, confidentiality, and dignity.Encourages independence in activities of daily living. Possesses a full understanding of fire, emergency, and safety procedures.Adheres to policies, procedures, and standards regarding infection control. Has a full understanding of the hazards and safety of oxygen being used by residents.Reviews any new faxes, fax new orders to medical doctors and pharmacy. Reviews all labs and x-ray fax copies to physician and reviews with RN.Checks list of residents scheduled for physician and other appointments and complete progress reports to be sent to physician with the resident.Re views narcotics count.Reads communication book and daily logbooks and address any concerns. Serves in a lead role to CMAs, CHHAs, and CNAs to ensure implementation of resident care plan and service plans. Documents in communication book properly. Ensures staff are reading communications and following through on communications as needed.Prepares, administers, and records medications in accordance with policy. Orders new medications as needed and document appropriately.Addresses any changes and review with RN prior to signing on monthly basis review new MARS.Ensures that CHHAs and CNAs complete monthly weights and vital signs as assigned.Reviews monthly weights and vital signs, reports any deviation from resident's norm to RN and MD, and documents appropriately.Makes weekly contact with each resident either in person or via telephone.Documents any unusual incidents, change in condition, new orders, change in medications in resident's medical record and communicate any concerns directly to RN.Participates in developing and implementing care plans and health service plans with RN as needed.Performs all the essential functions of a certified medical assistant as needed.Participates annually in continuing nursing education. Shows flexibility in working schedule to include evenings. Maintains CPR and First Aid certification.Develops and maintains written objectives, standards, procedures and organizational plan for delivery of nursing services.Maintains a working relationship with the caregivers, RN, DOW, and assisted living Administrator, and other Capital Health employees.Performs other duties as assigned.Minimum RequirementsCPR Requirements: Requires ACLS (or must obtain within 6 months of hire date) if assigned to: Critical Care/Intermediate/Telemetry, Emergency Rooms, Pediatrics/Pediatrics Emergency Room, Labor & Delivery, Surgical Services (not to include Perioperative), Interventional Procedures, Observation.Requires NRP (or must obtain within 6 months of hire date) if assigned to: Maternity Services, Emergency Room RMC/DeborahRequires PALS (or must obtain within 6 months of hire date) if assigned to: Emergency Rooms, Infant Follow-Up, Surgical Services (only PACU & Same Day Surgery), Pediatrics/Pediatric ED. ENPC accepted in lieu of PALs for Adult Emergency RoomEducation: Graduation from an accredited School of Practical Nursing. Current LPN license valid in the State of New Jersey.Experience: Two years of clinical and supervisory experience preferred. Training or previous experience in rehabilitative nursing preferred.Other Credentials: Licensed Practical NurseKnowledge and Skills: Has a knowledge and understanding of the Assisted Living Program rules and regulationsPHYSICAL DEMANDS AND WORK ENVIRONMENTFrequent physical demands include: Sitting , Standing , Walking , Carry objects , Push/Pull , Twisting , Bending , Reaching forward , Reaching overhead , Squat/kneel/crawl , Wrist position deviation , Pinching/fine motor activities , Keyboard use/repetitive motionOccasional physical demands include:Continuous physical demands include:Lifting Floor to Waist 10 lbs.
