Job Description of Licensed Practical Nurse:

Greenlife Healthcare Staffing is seeking a dedicated Licensed Practical Nurse to join a five-star rated Home Care Facility located in Brooklyn (Strickland), New York. This facility has consistently received top ratings from governmental agencies, reflecting its commitment to providing exceptional patient care.

Requirements of Licensed Practical Nurse:

  • Must have at least 6 months of clinical experience.
  • Must have an active New York state license
  • Candidate must have both CHHA and Home Care Home Base training.
  • Must have an active driver's license

Schedule of Licensed Practical Nurse:

  • This is a Full-time / Day Shift (Flexible hours).

Compensation and Benefits of Licensed Practical Nurse:

  • The salary for this position is $39.20 / hr.
  • $5,000 Sign-On Bonus
  • 3 weeks of vacation
  • 5 personal days
  • 8 sick days per year

Essential Job Duties of Intake Registered Nurse:

  • Ensure quality and safe delivery of services by taking referrals and processing appropriate referrals for home health care.
  • Contacts hospital discharge planner, primary nurse, physician, and case managers as needed to obtain complete referral information.
  • Develops and enhances relationships with referral sources.
  • Assesses the medical necessity of new referrals and arranges preadmission assessments as necessary to verify eligibility for home care.
  • Provides complete referral information to community health supervisors and designees.
  • Accurately takes physician or authorized practitioner's orders, verifies these in writing, and disseminates orders to appropriate health care personnel.
  • Completes all required documentation in an accurate, legible, and timely manner in accordance with established policies and procedures.
  • Involves physician or authorized practitioner, patients, and caregivers in developing plans of care whenever possible and appropriate.
  • Provides information regarding the patient's plan of care to the physician or authorized practitioner and other personnel as appropriate and in a timely fashion.
  • Completes, documents and submits work as per policy and procedure.
  • Attends meetings and case conferences as appropriate.
  • Communicates in a timely fashion to all applicable personnel all referrals and any changes in patient status.
  • Demonstrates understanding of and compliance with philosophy, objectives, and goals.
  • Gives accurate information to patients, caregivers, referral sources, and other staff members regarding services and associated costs. Demonstrates knowledge of reimbursement and regulation.
  • Completes, documents and submits work as per policies and procedures.
