Job Description
Aids the continuum of quality patient care throughout hospitalization and oversees provisions for patient's discharge. Helps in assessing and planning the appropriateness of care throughout admission and hospitalization of the patient.
Job Responsibility
- Administers medications and treatments and participates in nursing care management through delegation and directing nursing interventions. Collect data through direct observation/assessments, interviews, and record review and perform interim focused assessments addressing common reoccurring chronic health needs.
- Assist the RN in the collection of data to be used in regular assessments.
- Verify that all patients' monthly routine medications and other medications out of monthly cycle match the three-way verification medication check process.
- Address all assigned issues within allocated timeframe
- Perform health checks on a regular basis (bi-weekly, monthly, etc., as determined by the patient's team)
- Notes and schedule of all Doctors appointment and follow-up
- File all medical records according to ER discharge instructions and completing Client Health Records prior to annual physicals;
Job Qualification
- Graduate of an accredited school of practical nursing or of a Nursing Vocational or Nursing Technical school required, or equivalent combination of education and related experience.
- Current License to practice as a Practical Nurse in New York State required, plus specialized certifications as needed.
