Established in 1912, the Jewish Home of CNY evolved from a small rooming house in downtown Syracuse to become the 36-acre campus in DeWitt known as Menorah Park. Looking for an LPN to be the Director of our Adult Day Program. DUTIES AND RESPONSIBILITIES:

  • Ensures the provision of services to registrants including but not limited to medical, nursing, preventive, diagnostic, therapeutic, rehabilitative, recreational, psychosocial, and transportation.
  • Assumes lead role in marketing the Adult Day Health Services.
  • Ensure that there are written plans describing the philosophy, objectives, and program for providing and delivering non-occupant services to include budget preparation and monitoring.
  • Ensures coordination of services for registrants with services provided through other community agencies and avoids duplication of services.
  • Ensures a comprehensive assessment of the health, social related needs of all registrants to determine the range and kinds of services required.
  • Ensures adequate staff, space, and equipment necessary for the program.
  • Ensures that the philosophy and policies for services provided to the registrants are consistent with care and services provided to patients and residents.
  • Ensures the efficiency and economy and quality of operations.
  • Ensures review and evaluation of the effectiveness of the registrant services programs at least once year.
  • Ensures assigning and supervising activities of all personnel to ensure that registrants receive assistance in accordance with their needs.
  • Ensures the arrangement of in-service training and staff development.
  • Ensures securing the services of the registrant's personal, alternate or staff physician when need indicates.
  • Requirements:
    • Must be an Licensed Practical Nurse with NYS.
    • Must have Supervisory experience.
    • Experience working with people with developmental disabilities
    • Social Work experience preferred.
