Job Title: Licensed Practical Nurse - General & Geriatric CareJob DescriptionWe are seeking a dedicated Licensed Practical Nurse (LPN) to join our team. The ideal candidate will provide high-quality patient care, perform various medical tasks, and collaborate with healthcare professionals to ensure optimal patient outcomes. This role is essential in maintaining patient records, administering treatments, and supporting physicians during examinations.ResponsibilitiesInterview and maintain patient's medical records, including nursing observations, health history, and vital signs.Perform in-house labs and collect specimens for outside labs, ensuring quality control per organizational protocols.Prepare equipment and assist physicians during treatments and examinations.Complete and update assigned tasks and logs, such as recording shipments and vaccines received.Initiate follow-up actions as directed by the provider, including encounters and lab results.Provide health education based on physician's diagnosis and patient inquiries, referring to RNs as necessary.Complete prior authorizations for specialized medications and services.Attend case conferences and document case updates in patient records.Collect urine drug screens and follow up on patient adherence, documenting accordingly.Collaborate with community and referral resources.Participate in quality improvement measures to ensure chart compliance and best practices.Essential SkillsLicensed Practical Nurse (LPN) with a valid New York State license.CPR Certification.Infection Control Certification.0-4 years of experience, ideally in a primary care setting.Proficiency in health care and medical procedures.Additional Skills & QualificationsHigh school diploma or GED required.Preferred: Vocational/Technical School/Diploma Program in Nursing.Preferred: 2-4 years of related work experience.Work EnvironmentThe organization consists of community health centers throughout New York's Hudson Valley, NYC, and Long Island. Candidates will work 35-hour weeks, including 1-2 late nights per week. The site operates Monday from 9:00am to 8:00pm, Tuesday to Thursday from 8:00am to 8:00pm, Friday from 8:00am to 5:00pm, and Saturday from 8:00am to 4:00pm. The dress code is scrubs.
