Visiting Nurse and Hospice for VT & NHcovers more than 70 towns delivering superior nursing, rehabilitation, hospice, and personal care services with proven effectiveness, integrity, and compassion. Hospice Licensed Practical Nursesat the VNH are an integral part of the care team, where they assess a patient's health status and complete nursing interventions in collaboration with the interdisciplinary team members and the patient. As a member of the hospice team, the Licensed Practical Nurse (under the direction of an RN Case Manager) is responsible for direct nursing care to Hospice patients and for educating patients and families in accordance with the care plan established by the interdisciplinary team.

The LPN will:

* Making home visits, under the direct supervision of the RN Case Manager, to assist with the provision of care to meet patient/family needs.

* Communicates changes in patient condition and needed changes to the plan of care to appropriate team members and caregiver.

* Assists the RN and/or medical provider in performing specialized duties related to end-of-life care.

* Observe, document, and report to the RN as appropriate, the patient's physical and emotional needs and reactions to care.

Minimum Qualifications:

* Must be a Licensed Practical Nurse in Vermont and New Hampshire.

* A graduate from an approved school of practical nursing.
