LPN will perform tasks and responsibilities within the framework of case finding health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse.
Schedule is Mon-Fri 8:30-5pm
Job Responsibilities:
- Calls the home care coordinator, RN, or supervisor with verbal report of initial findings.
- In conjunction with the family and physician and/or authorized practitioner and Americare Supervisor adheres to the Plan of Care based on the RN s assessment.
- Provides nursing care as outlined in the Plan of Treatment to include the following: Nursing services, treatments and preventive procedures including those requiring substantial specialized skill and ordered by the physician or authorized practitioner.
- Initiates preventive and rehabilitative nursing procedures as appropriate for the patient s care and safety.
- Evaluates the effectiveness of his/her nursing service to the individual and family.
- Observes signs and symptoms and changes in the patient s physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary.
- Provides and completes nutritional assessments under the direction of Director of Nutrition.
- Reports all patient changes and information to the Primary RN, Supervisor or Case Manager of home care on a timely basis.
- Submits timely records of visits and vouchers based on each visit
Job Qualifications:
- NYS LPN License.
- CHHA Experience preferred.
- Home care experience a plus.
