Job Title: Licensed Practical NurseJob Summary:CASES is seeking an LPN to work on our Nathaniel ACT (Assertive Community Treatment) team. The ACT Teams provide non-traditional, person-centered and recovery oriented community based mental health treatment to promote the wellbeing and community integration of adults with serious mental illnesses and co-occurring substance use disorders. The Nathaniel ACT Team operates as an Alternative to Incarceration program working with participants who are convicted of felony crimes. The LPN is an integral part of the team, working under the direction of the full-time RN providing medication management, health assessment and counseling to promote consumer recovery, wellness, and optimal health. The nurse provides services within the framework of core ACT operating principles, professional nursing ethics, and State and City regulatory guidelines. The LPN conducts services in the community as needed to support consumers to have needed access to medications and healthcare. Our office is easily accessible by public transportation.Workplace Flexibility: Flexible - Staff have flexibility to complete their work day from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work.What You Will Be Doing:
- Provides at least 80% of medication management and health services to consumers in the community, including but not limited to: medications administration (both oral and intramuscular), monitoring medications side effects and adherence, phlebotomy, coordination of consumers' health care services, providing psychoeducation to consumers about health and medications, assessing consumers' mental statuses;
- Completes and signs progress notes, incident reports, and other required documentation in accordance to the New York State Office of Mental Health (OMH) regulations, the New York City Department of Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards and regulations;
- Coordinate with FACT RN and prescriber on consumers' medication management needs, health care services, maintaining consumers' medication administration schedules;
- Coordinates care with family and other community service providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providers;
- Participates in daily team meetings, clinical meetings, and monthly nurses meetings to collaborate and discuss clinical issues and clients' needs with team members and F/ACT nursing staff;
- Use motivational interviewing, culturally and developmentally appropriate, recovery oriented and trauma-informed approaches when delivering services;
- Any other duties as required by Team Leader and senior clinical leadership.
What We Are Looking For:
- Licensed Practical Nurse (LPN) with current New York State license.
- At least three years of experience working with adults (ages 18 and up) in one or more of the following areas: psychiatric nursing, substance use treatment setting, social service settings caring for individuals experiencing homelessness, justice involvement or limited social support.
- Knowledge of community treatment, support services, and resources.
- Highly organized and self-motivated.
- Proficient in computers.
