  • Mon- Fri 9am-5pm OR 8am-4pm; 35 hours/week- 1 opening
  • 21 hours/week- 1 opening

Salary: $60k for full-time; Pro Rated on $60k for part-time

  • Caseload: 34 clients in each team, shared with RN
  • Team mix: Clients: SMI/High Risk; Staff: MD/PMHNP/Masters level clinicians/Peers/Case Managers.
  • Overview

    The Assertive Community Treatment (ACT) team at The Bridge delivers innovative, community based mental health care to support individuals with serious mental illness and co-occurring substance use disorders who have been charged with a felony. The LPN plays a critical role in promoting recovery, wellness, and optimal health, providing health assessments, counseling, and medication management. Services are offered both in the community and at the ACT office, following core ACT principles, nursing ethics, and regulatory guidelines. This is a field-based role, with 80% of services delivered in the community.

    Responsibilities

    • Develop and uphold nursing practice standards within LPN scope.
    • Assess client needs for housing, mental health treatment, medical care, and support services.
    • Advocate for clients to secure medical care and community resources that support integrated health.
    • Collaborate with the RN to manage medication inventory and medication administration.
    • Coordinate medical care with the ACT psychiatrist and ensure clients have a primary care provider.
    • Administer medications, vaccines, PPD tests, and perform phlebotomy.
    • Monitor vital signs, assess medication side effects, and report findings to the team.
    • Maintain accurate documentation in the Electronic Health Record (EHR) and manage schedules for prescriptions.
    • Provide culturally competent services in accordance with The Bridge's policies. Other duties as assigned by the ACT Director and Administrators.

    Qualifications

    • Current New York State Licensed Practical Nurse (LPN) license.
    • At least three years of experience working with individuals facing mental health, substance use, homelessness, or criminal justice challenges preferred. Knowledge of community treatment, resources, and support services.
    • Strong organizational skills, self-motivation, and computer proficiency.
    • Fluency in Spanish preferred.
