Description

We are seeking a dedicated Licensed Practical Nurse to join our primary care clinical team! The ideal candidate is motivated, exceptionally organized, and able to anticipate what will contribute to a positive visit for the patient and family. The LPN will provide direct patient care under the supervision of a registered nurse or physician.

Responsibilities:

* Screening and updating patient health maintenance and wellness indicators

* Monitor and record In Electronic Health Record patient vital signs and medical information updates

* Promoting flow of services during visits, coordinating education and resources

* Performing point of care testing as required

* Setting up Tele-health and remote patient visits

* Collaborate with interdisciplinary teams to provide comprehensive patient care

Qualifications

Requirements:

* Current LPN License in MA

* Strong understanding of nursing standards and regulatory requirements

* Excellent communication and interpersonal skill

* EPIC EHR as well as outpatient Primary Care experience a strong plus
