Overview The LPN reports to the Nurse Manager, Ambulatory Care. Responsibilities
- Utilizes observational skills to determine patient's total needs.
- Gathers data concerning the patient's physical status through inspection and discussion with the Registered Nurse and/or Physician.
- Assists in collecting pertinent information for completing initial and ongoing screenings e.g. depression/anxiety screening.
- Able to identify common recurrent patient problems, symptoms and behavioral changes in relationship and interactions with others.
- Assists the RN with discharge, interventions/treatments and education of patients at each patient encounter.
- Provides input to the patient care team to establish an interdisciplinary plan of care for each patient.
- Ensures that all patient care equipment is available and functioning by performing testing as per policy.
- Updates initial nursing plan of care as patient condition/needs change in collaboration with registered nurse.
- Administers medication if needed and documents patients' response to medication administered.
- Identifies expired floor-stock medications, and sends back to pharmacy.
- Utilizes sound clinical skills and interventions in the provision of nursing care delivery.
- Maintains standard of care and demonstrates competency in performing all procedures within prescribed standards.
- Provides health teaching and counseling to patients, caregivers and significant others under the direction of the RN.
- Demonstrates knowledge of medications and their correct administration based on age of the patient and the patient's clinical condition.
- Demonstrates the ability to assist physicians with procedures and performs services requiring technical and manual skills under the direction of an RN
- Observes Infection Control policies and procedures, and participates in activities to evaluate infection control practices.
- Demonstrates initiative by identifying potential safety, or inflectional control issues and makes recommendations for improvement.
- Performs patient care responsibilities considering the needs specific to the standard of care and practice for patients.
- Takes appropriate actions and interventions in the management and care of patients during an emergency or urgent situation and shows responsibility in keeping patients calm and informed regarding the situation.
- Performs basic life support and follows the policy/procedure for the management of a patient needing basic life support.
- Guides and assists unlicensed nursing personnel in tasks appropriate to their role.
- Observes safe work practices and promotes a safe environment for self, patients and other employees. Shares with appropriate persons any issues affecting patient and/or staff safety.
- Participates in performance improvement activities and works toward ways to improve own area to achieve a positive outcome.
- Treats patients and their families/significant others with respect and dignity and ensures patient confidentiality.
- Participates in Quality and Performance Improvement activities.
- Other duties as assigned.
Qualifications Education:
- Graduation from an accredited school of practical nursing or completion of equivalent study in an approved program in professional nursing.
- Some college preferred.
Experience:
- Experience in outpatient care setting and experience with care plan development, including goal setting
Knowledge and Skills:
- Ability to communicate in English, both verbally and in writing.
- Basic computer knowledge, and word processing skills.
- Good written, and verbal communication skills.
- Ability to use verbal, and non-verbal calming techniques to prevent crisis situations.
License, Certification:
- Current NYS Licensure or the eligibility to be licensed to practice as a practical nurse.
Physical Demands:
- Involves walking, sitting, talking, listening, lifting and bending.
- May include some repetitive motions.
