POSITION SUMMARY:

The LPN provides individualized nursing care and comprehensive services to patients and families in their own homes under the supervision of a Registered Nurse.

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

• Provides basic patient care under the supervision of a Registered Nurse in accordance with physicians orders
• Observes patients, charting and reporting changes in patients conditions, such as adverse reactions to medications or treatment and taking any necessary action
• Educates patient and families in self-care and health management
• Attends case conference as requested
• Adheres to policies and procedures
• Participates in agency quality management and safety activities

Educational Requirements and Experience:

• Graduate of an accredited school of nursing
• Current license to practice as a practical nurse in the state of Massachusetts
• Current CPR required
• Two years nursing experience
• Current MA drivers license and reliable transportation
