Post Internal Days 0 Category Unaccompanied Alien Children (UAC) DescriptionCATHOLIC GUARDIAN SERVICESJOB TITLE: Licensed Practical Nurse (LPN)DEPARTMENT: Family Support Services REPORTS TO: Healthcare Coordinator OBJECTIVE OF POSITION To ensure that appropriate medical and dental care as required by NYC and NYC regulations and as dictated by good health care practice is provided to all children assigned to the UC program. ESSENTIAL RESPONSABILITIES 1. Coordinates, monitors and documents the health care of the children in his/her assigned caseload. Activities include, but are not limited to:
Schedules and obtains the documentation for the initial physical exam Provides basic healthcare treatment Tracks periodic health and dental exams. With program case manager, notifies foster parents/youth care worker of examinations that are due and follows up with foster parents when appointments are failed and need to be rescheduled. Coordinates referrals made to specialty clinics and follows up any recommended tests, treatments or therapies. Follows up incident/accident reports, emergency room visits, and sick visits to ensure that the prescribed medical regimen is being carried out. Reviews and documents all medical and dental reports and coordinates follow-up as directed. On a case by case basis, completes or coordinates completion of forms required for specific purposes. 2. Staffs and assist in coordinating the agency's Well Care Clinic Prior to each clinic session reviews medical record of children scheduled to be seen Interviews child about child's health history and current health status; measures vital statistics, screens vision and hearing; and administers immunizations prescribed by pediatrician. Following clinic session, reviews completed examinations forms, records pertinent information in chart and in portal, and complete and forwards to Assistant Program Director and Case Manager any recommendations for follow-up care. 3. Educates the children program and colleagues regarding health and safety issues. Serves as a resource to casework and administrative staff concerning the health status of his/her clients When indicated, confers in person and /or on the telephone with children, foster parents about the health care needs and medical management of the children in his/her caseload 4. Maintains a close working relationship with the community-based health care facilities that provide the majority of health-related services to UC children. On a regular basis consults with staff of those facilities to educate them about foster care issues, to discuss and solve any problems and obstacles to UC children accessing appropriate care, and to enhance communication between all parties involved. 5. Attends training sessions (both in and out of agency) as requested by health care coordinator. QUALIFICATIONS /SKILLS LPN Minimum three years of nursing experience, preferably in pediatrics. Current holder of nurse's professional liability / malpractice insurance.
