Compensation: Competitive pay scale - $41.00 - $42.00/hour

Role Highlights:
- Provide in-home patient care and monitoring
- Collaborate closely with Advanced Practice Providers (APPs) to administer directed treatments
- Maintain precise and compliant medical records using our HealthPoint and Athena EMR system
- Access synchronous telemedicine support as needed for patient care
- Engage in meaningful conversations about patient well-being, medication management, and care goals
- Adhere rigorously to clinical standards, protocols, and performance metrics
- Deliver care with a deep sense of empathy and cultural sensitivity
- Champion a patient-centered approach to healthcare
Position Details:
- A variety of full-time and per diem positions are available
- Work schedule offers flexibility with alternating weekends (10:00 AM - 8:00 PM)
- Opportunity to utilize and enhance your clinical skills in a unique healthcare setting
Qualifications:
- Minimum of two years of LPN experience
- Current and valid American Heart Association BLS certification (ACLS certification is a plus)
- Previous experience in Emergency Department, Urgent Care, Internal Medicine, Hospital - Critical Care, Family Practice, or related fields is highly desirable
- Active and valid driver's license with a minimum of 2 years of driving experience
- Current unrestricted NY State LPN license
- Clinical experience with pediatrics is a valuable asset
- Completion of third-party Phlebotomy training is advantageous
