POSITION PURPOSE:
The Licensed Practical Nurse (LPN) assists the Medical Provider or Registered Nurse (RN) in the care and treatment of Health Partners of Western Ohio's patients. The nurse performs nursing services utilizing the nursing process. The LPN shall perform a variety of complex nursing tasks and related duties as required in a patient centered medical home setting.
QUALIFICATIONS
EDUCATION/CERTIFICATION: Graduate of a school of nursing accredited by National League of Nursing.
Current license to practice as a Licensed Practical Nurse in the State of Ohio.
REQUIRED KNOWLEDGE: Successful completion of in-house training.
EXPERIENCE REQUIRED:
SKILLS/ABILITIES:
* CPR - BLS (basic life support) required.
* One to three years full-time experience in nursing practice is preferred.
* Ability to work as a team member.
* Computer experience desirable.
* Ability to work with a culturally diverse group of people.
* Must be willing to work flexible hours as required by position.

ESSENTIAL FUNCTIONS AND BASIC DUTIES:
* Collects patient data through physical examination techniques, interview, and observation.
* Documents all nursing care provided according to policy.
* Formulates plan of care for clients under direction from providers.
* Obtains referrals from other physicians and specialists.
* Administers medications as prescribed by the provider.
* Follows approved protocols/standing orders for client care.
* Follow universal precautions.
* Performs treatments, procedures as prescribed by the provider.
* Performs technical skills.
* Responds to client/family/caregiver needs.
* Responds to crisis situations according to policy.
* Utilizes client education for emotional and physical health maintenance.
* Utilizes supplies in a cost-effective manner.
* Performs clinic and outreach related tasks.
* Utilizes on-going data collection skills.
* Participates in Quality Improvement activities.
* Demonstrates effective communication skills.
* Establishes and maintains professional working relationships with other members of the health care team.

* Maintains stocks of medicines and medical supplies as necessary.
* Educates and advises patients on specified medical issues within established parameters.

* Completes referrals and schedules tests for patients.
* Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor or suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices.
