Job Duties
PATIENT CARE
Collects data from patient, family or significant others and records on Initial Data Base form.
Contributes to the formulation and updating of plan of care and implements plan of care under the direction of the Registered Nurse.
Gives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes) according to established standards.
Recognizes changes in patient condition and responds appropriately, notifying charge nurse as needed.
Documents pertinent information regarding patient's responses and nursing interventions in patient's medical record.
Administers medication according to hospital policies.
PATIENT CARE RELATED DUTIES
Applies principles of infection control and safety in the performance of patient care.
Creates and maintains a therapeutic milieu and facilitates nursing groups.
Communicates effectively with charge nurse and other member of the health care team regarding patient's needs and/or changes in condition.
Responds appropriately in crisis situation.
Reports significant unit problems to the Registered Nurse.
Patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served.
Orients patient and family to new environment; develops rapport with patients family and/or significant others.
Orients LPNs and mental health assistants as required.
Effectively utilizes the hospital computer system as it pertains to job responsibilities and in accordance with hospital policy.
Participates on unit committees and in CQI activities as assigned.
Participates in patient care conferences and in staff development activities as required.
Assists other personnel as needed.
SkillsAmerican Heart Association Basic Life Support certification required.Clinical nursing skills appropriate for an LPN.Good oral and written communication skills.Good interpersonal skills.Speaks, reads and writes English to the extent required by the position
Education/LicensureGraduation from an accredited school of Practical Nursing or equivalent education as described by the State Education Law.Current NJ license as an LPN.Successful completion of Progressive Care Orientation Program required.

JOB SETTING/PHYSICAL DEMANDSAll patient care areas; contact with patients, visitors, staff.Frequent prolonged standing/walking.Lifts, positions, pushes, and/or transfers patients.Protective clothing worn: face shield, gloves, impermeable gown, and particulate respirator.
