The ideal candidate will have a strong background in nursing care, excellent communication skills, and the ability to work independently. This role involves providing direct nursing care under the supervision of a Registered Nurse (RN) and following established care plans. You will also be responsible for documenting patient progress, administering medications, and educating patients and their families.Key Responsibilities:
- Provide health education according to the teaching plan established by the RN.
- Deliver routine nursing care as outlined in the nursing care plan and within the LPN scope of practice.
- Document services provided, changes in patient condition, and other pertinent observations.
- Measure and record vital signs and collect routine patient data.
- Identify high-risk situations and make appropriate referrals.
- Administer medications accurately and transcribe physician orders.
- Adhere to infection control policies and maintain a safe environment.
- Assist in the movement and transportation of patients when necessary.
Qualifications:
- Education: Graduate of an accredited school of Practical Nursing.
- Experience: Minimum of 1 year as an LPN.
- Licensure: Current Practical Nurse license in the State of New York.
- Skills:
- Strong computer skills and proficiency with EMR systems.
- Excellent interpersonal, organizational, time management, and communication skills.
- Ability to complete paperwork accurately and in a timely manner.
- Knowledge in measuring and administering medication.
- Requirements:
- Must comply with HIPAA and privacy regulations.
- Ability to lift a minimum of 50 lbs and stand or walk for long periods.
- Must possess a valid NYS driver's license and a reliable vehicle with insurance.
- Willingness to travel throughout the assigned county.
- Must be willing to relocate to Rensselaer, NY before starting.
