Job DescriptionJob Summary:The incumbent provides routine clinic nursing procedures and personal care services to patients.Essential Functions: Obtains and records patient's vital signs and weightAssists Provider with patient examinations perform medical procedures, and wound care, as neededAdministers and documents medication administration in patient chart example oral subconscious or intramuscular injectionCollects samples, such as urine, blood, and sputum, from patients for testing and performs routine laboratory tests on samplesReviews Provider's orders, lab requests and close out tasks and provider or nurse and LPN bucketsInterviews patient to determine medical problem/condition and documents in chart for providerAssist patients with scheduling follow appointmentsComplete and submit prior authorizations for imaging, medications, Nexplanon, Paragard, proceduresProcesses patient referral requests and Conducts insurance Verification for referralsFollow up on patient request as ordered by provider example medication refills telephone inquiries from affiliated agencies and other specialty providers as neededMaintains health department STD treatment lists and verification recordsCommunicate with affiliated agencies regarding test results and updates on patients, maintenance of Health Department. STD Tx lists and verification of TxSupports Chronic Care Management TeamAssist with stocking and ordering new supplies and disposing of expired or damaged suppliesAssist in administering Covid-19 testing and educationSterilizes equipment and suppliesMaintains crash cart to ensure all medications and supplies are currentAct as back up for patient flowMeeting with pharmaceutical representative and receiving medications and aiding with connecting with providersOversees Vaccine for Children (VFC) ProgramMaintains medical equipment and monitors need of equipment.Assist with onboarding on new CMAsParticipates in community education programs and Health fairsAdministers and coordinate flu vaccine clinicsProvides education to patients as neededPerforms other duties as assignedSupervisory Functions: NoneExperience and SkillsQualifications:One year certificate from college or technical school for Licensed Graduate Practical Nursing; One year of practical experience in a community health center or an allied health field environment.Strong customer service focusEffective verbal communication skillsTeamwork orientationOrganized and able to manage competing prioritiesGood judgmentResourcefulness in problem solvingAble to take and follow through with delegated tasks and accountability
