Job Title: Full-Time LPN (Licensed Practical Nurse) - Permanent PlacementLocation: 30-Bed Skilled Nursing Facility (SNF), 5-Star Rated Facility Shift: 5:00 PM - 5:30 AM, 3 Shifts Per Week (Every Other Weekend & Some Holidays Required)Pay Range: $25 - $30 per hour (based on experience), Up to $43/hr. for Shift Pick-ups Job Description: We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a full-time, permanent position at our 5-star rated, 30-bed Skilled Nursing Facility (SNF). As an LPN, you will play a key role in providing high-quality care to our residents while working the 5:00 PM to 5:30 AM shift, three days per week, with every other weekend and some holiday shifts required. - Provide high-quality nursing care to residents in accordance with care plans and facility policies.
- Administer medications, treatments, and provide general nursing care to residents.
- Monitor resident conditions, document observations, and report changes to the healthcare team.
- Collaborate with doctors, RNs, CNAs, and other healthcare professionals to ensure optimal care.
- Maintain accurate medical records using the PCC EMR system.
- Ensure a safe and comfortable environment for all residents, following all infection control guidelines.
- Assist in resident assessments, care planning, and evaluations of progress.
Qualifications:
- LPN license required (current and valid in the state).
- Minimum of 1 year of LTC experience is preferred.
- Experience with PCC EMR system is highly preferred.
- Strong communication skills and ability to work as part of a collaborative team.
- Compassionate, patient, and dedicated to providing excellent care to residents.
- Ability to work 3 shifts per week with every other weekend and some holidays.
Compensation & Benefits:
- Hourly pay: $25 - $30 per hour, based on experience.
