Description
Pay Range: $20.50-$23.50 per hour
Duties and Responsibilities
Administer medications to residents in accordance with the Residential Medication policies and procedures
Accurately and consistently complete all required documentation practices
Assist in the personal and physical care needed by each individual as stated in their residential habilitation plan (including tasks such as bathing, toileting, eating and dressing) if needed
Performing housekeeping tasks as designated by the shift supervisor/Residence Coordinator (including tasks such as meal preparation, cleaning and laundry) if needed
Attend and participate in all team/program meetings and other meetings as requested
Follow the Code of Ethics for Direct Support Professionals, as well as, show proficiency in the Core Competencies to ensure person-centered supports for the residents
Implement fire, safety and emergency procedures and assist with fire drills
Implement and follow all required individual programming and Agency/Program-specific policies and procedures accurately, consistently and according to how they are written
Assist in transportation to and from medical appointments as required
Maintain Agency-Approved Driver status and completes all required ongoing/annual trainings and in-services, as assigned
All other duties as assigned
Minimum Qualifications
Licensed Practical Nursing licensure
6 months to 1 year experience working with individuals with developmental disabilities
Ability to read, write, speak and understand English
Possess a current and valid NYS drivers license with an acceptable driving record
Be able to regularly lift and/or move up to 50 pounds
Preferred Qualifications
Experience in an OPWDD certified residential setting
Certifications and Trainings
Must maintain LPN Licensure
NYS Medication Administration
CPR
First Aide
SCIP-r
All other trainings mandated by the OPWDD
