Reports to: Departmental Lead, Supervisor or Manager Job Type: Full-time, 40 hours per week with on-call responsibilitiesLocation: Elmsford, NY (territory to cover Manhattan, Bronx, Westchester and North Elmsford, NY)Requirements LPN LicensedValid Driver's licenseClean Driving record Must have at least one year of home care, critical care, LTAC or long-term care experienceMechanical Ventilator and Pediatric experience is a major advantageMust have solid working knowledge of charting in EMR and Must have a general knowledge of medical equipmentJob Responsibilities, included but not limited to:Data entry and scanning into patient charting system Updates visit spreadsheets, and shared document files as directedCompletes Non-Clinical Follow-Ups and completes Documentation in EMRDocument and maintains updates for all patient visitsConducts in-person monthly or quarterly follow-up visitsExchange Medical equipment under the supervision of the respiratory therapistParticipates in the On-Call ProgramMust follow infection control practices and PPE policies established by the companyOther duties as assignedPhysical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
