Informal leaders who care enough to give their very best, lead by example, and be a mentor to teammates and nursing assistants. Responsibilities: PUBLIC RELATIONS:
- Cooperates with other department managers in planning activities that bring the community to the facility (holiday meals, etc.).
- Maintains good relations with physicians, clinics, social worker, etc.
- Understands that she/he represents Eldercare of Bemidji in every exchange and strives to do so in a positive manner.
- Understands that excellent customer service is the highest form of public relations and her/his role in providing the highest level of customer service.
RESIDENTS:
- Reports Vulnerable Adult issues to the Administrator, and DON, County and Ombudsman as needed.
- Understands and complies with the responsibilities of a mandated reported for vulnerable adults.
- Understands and observes the need for strict resident confidentiality
- Administers personal hygienic and nursing care, medications, treatments and therapeutic measures as directed by physician's orders and according to facility policies and procedures and the boundaries of their license.
OPERATIONS:
- Works as a team member and assists with implementation and continuing evaluation of residents plan of care, emphasizing rehabilitative nursing and restoration of the resident to his/her full potential for living.
- Observes and records symptoms, reactions and changes in residents condition including general physical and mental condition and reports findings to Registered Nurse.
- Assists in providing emotional and physical comfort and safety for residents.
- Prepares and cares for equipment and supplies with an understanding of principles and methods of handling sterile material.
- Is responsible for following instructions issued by the Director of Nursing and R.N.'s and for adhering to established facility routines and policies and procedures.
- Has complete understanding of basis of infection control and universal precautions to avoid infection of residents and self or contamination of equipment and supplies.
- Maintains good housekeeping standards in all areas.
- Adheres to all safety measures and conveys positive attitude toward safety of self, residents, coworkers, equipment and supplies.
- Able to control negative emotions and provide care in a therapeutic manner.
- Attends appropriate in-services, facility education programs and staff meetings.
- Has working knowledge of fire regulations and disaster procedures according to facility policies and procedures.
- Reports to work as scheduled and consistently demonstrate dependability and punctuality.
- Utilizes computerized medical record effectively.
- Utilizes wireless call system effectively.

BUDGET:
- Participates in control of costs through proper use of equipment, appropriate use of supplies and effective, efficient organization of duties.
- Understands the correlation between census and income and that expenses must be reduced in times of low census including adjustment of staff hours.
- Participate in the control of cost-effective services that include proper use of equipment, appropriate use of supplies, and effective, efficient organization of resident care duties.
BUILDING:
- Reports safety and environment hazards.
- Responsible for maintaining work areas and department in a neat and clean manner.
- Responsible for helping to maintain the facility in a safe and clean manner by picking up debris on the floors and wiping up spills.
- Aware of role in disaster plans for the facility and participates in these plans and procedures during drills and actual events.
- Responsible for reporting the need for repairs using the Maintenance Order form.
EMPLOYEES:
- Monitors Nursing Assistant work performance in providing direct resident care. If nursing assistant is not performing according to facility standards will act as coach/trainer to assist in assuring compliance with written policies and procedures. Should coaching assistance be unsuccessful, the LPN will report findings to the supervising RN.
- Insures proper documentation is provided for inclusion in each employees personnel file.
- Completes staff performance appraisals according to facility policy.
- Wears Name Tag at all times.
- Reports to work on time each day.
Requirements Job Qualifications:
- Current MN License as a Licensed Practical Nurse
- Preference given for experience in a geriatric long-term care setting.
- Work effectively with minimal supervision, strong communication skills, and ability to lead a group of people in completing tasks.
- Knowledge of standards of nursing practice and awareness of standards for compliance with state and federal regulations.
- Time to reach job competency is expected to be six months with at least one year of previous long term care experience. Entry-level staff are expected to reach competency in 1 year.
