A medical services organization in New York City is looking to add a new Licensed Practical Nurse (LPN) to their team in across the Metro area. In this role, the Licensed Practical Nurse (LPN) will be responsible for providing excellent care for monitoring, assessing, and providing directed treatment common to complex injuries and illnesses through comprehensive assessments, including diagnosis, treatment, and outcome management for adult and pediatric patients..
About the Opportunity
- Start Date: ASAP
- Setting: Clinic
- Schedule: 5 shifts per week (Monday to Friday, MUST work 2 weekends per month)
- Hours: 10-hour shifts, starting at either 8:00 AM or 10:00 AM.
Responsibilities:The Licensed Practical Nurse (LPN) will:
- Act as a conduit for the medical treatment of the Health Plan Partnerships patient population
- Navigate conversations related to management of health, well-being, medication management, and goals of care
- Provide supervised, directed treatment of common medical diagnoses
- Participate in continuing education as required by the state where you reside and the training team
- Adhere to clinical and safety standards, protocols, and performance metrics
- Provide care with cultural sensitivity and compassion
- Lead your practice with a patient-centered approach
- Other related duties as deemed necessary
Qualifications:
- Current, unrestricted NY State LPN license required.
- Active American Heart Association BLS certification required.
Desired Skills:
- Minimum of 1 year of clinical experience (home care, field experience, public health, outpatient settings, hospice, etc.).
- Phlebotomy experience required.
- Clinical experience with pediatrics and vital sign assessments is preferred.
- 2-3 years of experience in outpatient, hospital, or critical care settings is a plus.
- Ability to work in a home healthcare or community-based care setting is highly desirable.
- Strong communication skills to interact effectively with patients and families.
- Physical ability to lift 50-60 lbs.
- Ability to work independently while being supported by telehealth.
