The LPN will be responsible for delivering exceptional nursing care along side our RN's to our residents with developmental disabilites in a residential setting.

FULL DESCRIPTION
Responsible to: Director of Nursing
Job Summary: The Licensed Practical Nurse is an integral part of the treatment team in the site. The Licensed Practical Nurse is responsible for assisting the RN Case Manager and the site Management team to ensure that each individual's medical needs are met and that required medical follow up is
completed.
Essential Job Duties:
Assists the RN Case Manager in all aspects of the medical follow up for the individuals served.
Performs case finding, reporting observations to the site RN or RN On-Call.
Administer medications for all individuals during the scheduled shift.
Administer treatments, supplements, G-tube feedings, as needed during the assigned shift.
Document as per policy in individuals' charts.
Initiates Medical Monitoring as per RN/MD Directive and ensure completeness per directives.
Float to department sites as needed.
Completes medication orders with the pharmacy ensuring that all MD orders/scripts are up to date, available on site and are not expired.
Participates in the weekly site team meeting to ensure ongoing communication with the RN Case Manager/PC/RM.
Assists the RM with the daily MAR (Medical Administration Record), BM (Bowel Management), intake and elimination record check completion.
Accept med delivery, confirm meds delivered on order forms, write control sheets for control meds, be sure enough med available if not delivered
Assists with individual care, as necessary.
Upon individual's return from appointment, ER visit or Hospital discharge, will ensure that all recommendations, order changes are followed.
Direct shift staff to complete tasks as necessary.
Any other duty as assigned by supervisor

Must be a graduate of a NYSED approved Nursing Education Program

Additional Information:
Hourly rate based on relevant years of licensed experience
Schedule:Monday-Friday withalternating working a weekend day every other week
6am-2pm/Full Time
1:30pm-9:30pm/Full Time
3pm-8:30pm/Part Time
Physical Requirements:
The following physical requirements must be evaluated by a physician prior to entry into a Nursing program. Upon admission to the Nursing program, you will be required at that time to have a physical and have a doctor complete our required form, as to your abilities with regards to these physical requirements.
Strength: Sufficient to assist with lifting and transferring a patient and perform CPR.
Mobility: Sufficient mobility to bend, stoop, and bend down to the floor; ability to move around rapidly; and to move in small, confined areas.
Hearing: Sufficient to hear through the stethoscope to discern sounds; to hear cries for help; to hear alarms on equipment and emergency signals; and various overhead pages.
Vision: Sufficient to make physical assessments of patients and equipment.
Communication: Able to communicate in both verbal and written formats; and interact with clients, staff, and faculty supervisors.
