- Provide skilled nursing care to clients in their homes, including administering medications, wound care, and monitoring vital signs.
- Collaborate with families and other healthcare providers to develop and implement personalized care plans.
- Offer emotional support and companionship to clients, ensuring their physical and emotional well-being.
- Document patient progress and communicate any changes to the healthcare team.
- Educate families and clients on managing health conditions and improving overall quality of life.
- Contribute to hands on Care Professional training and assist with monitoring scope of service compliance
Qualifications:
- Current LPN license in South Dakota
- At least 2 years of experience in a healthcare setting (home health care preferred)
- Ability to work 2 weekends per month
- Strong interpersonal and communication skills
- Compassionate, empathetic, and patient, with the ability to work independently and as part of a team
- Reliable transportation and willingness to travel to client homes
