ResponsibilitiesThe Licensed Practical Nurse is responsible and accountable for the delivery of outpatient nursing care to patients care under the direction of a physician. Job duties including: demonstrates awareness of and sensitivity to patient's rights. Maintains dignity and confidentiality of the patient; administers medications and immunizations with scope of practice and under the direction of a physician; observes, measures, records, and reports data relating to a patient's health status. Provides clinical guidance to unlicensed care staff, such as Certified Medical Assistants. Identifies patient goals for consideration by the physician for possible inclusion in a patient's care plan. Cooperates with care team in the coordination of patient care activities. Maintains effective communication with staff, physicians, patients and families. Assists the physician with history taking, medication reconciliation and other intake/discharge activities. Provides clinical education to patients and their families.Communicates results and care plans to patients as directed by supervising physician.
Participates in performance improvement activities. Maintains a safe environment for patients and staff, applies appropriate precautions and protocols. Identifies and documents major changes in patients' condition and basic care needs, institutes appropriate interventions. Provides assistant to other specialty areas upon request.Pursues appropriate continuing nursing education. Assumes other related responsibilities when assigned. Assist in emergency situations
QualificationsGraduate of NYS Licensed Practical Nurse Degree Program . High school diploma or GED required. Currently licensed as a Licensed Practical Nurse in New York State and BLS Certification required.
