Ditmas Park Nursing & Rehab -
The Licensed Practical Nurse (LPN), is responsible for providing basic medical attention to Patients and assessing their wellbeing as part of a larger medical team. Their duties include checking vital signs, recording a Patients medical history and assisting Patients with hygiene.
Duties and Responsibilities:
- Administer medications and nursing procedures to Patients as prescribed.
- Obtain and record Patient temperatures, blood pressures, pulse rates, and other vital signs using standard instruments and procedures.
- Observe, monitor, and record Patient symptoms and conditions to obtain and provide information for diagnosis and treatment; identify responses to medication or changes in health status, and reports status to a registered nurse, physician, or other licensed health care provider.
- Prepare Patient for or assists with examinations and treatments.
- Maintain charts and other records of Patient conditions and/or administered treatment.
- Educate and counsel Patient regarding their health, medications, and treatment goals and procedures, as appropriate.
- Monitor supply inventories, and completes requisition forms to ensure adequate supply levels.
- As necessary, observe and supervise the activities of unlicensed direct care staff, including the administration of medication, to ensure compliance with applicable requirements and report observations to higher-level staff.
Education / Certification
Currently registered in New York State or has possession of a valid permit to practice as a Licensed Practical Nurse.
Demonstrates knowledge of the nursing processes.
Experience:
- Previous Long term Experience in LPN role is preferable.
- Good investigative skills.
- Ability to write, educate and work independently.
