The LPN functions under the supervision and guidance of the Physician and participates in the multi-disciplinary process of providing care and treatment to patients and provides clinical support and technical assistance to the professional medical and other staff. The LPN is responsible for functioning within the scope of practice dictated by the authority of the New York State Department of Education. Individual must be able to manage demanding workload with accuracy. Position requires excellent customer service skills with patients, and their families, other staff, physicians, vendors, and the public.
DUTIES AND RESPONSIBILITIES
- Assists physicians with patient office visits, including rooming patients, recording vital signs, reconciling medication lists according to provider protocols, and enters accurate past and current medical history into patient's electronic medical record.
- Administration of medication/vaccines in accordance with physician orders and The Five Rights of Medication Administration.
- Performs select clinical tests, such as glucose finger sticks and A1C testing
- Performs administrative task including but not limited to; completion of forms, prescription refills according to physician protocols, sending referrals, communication with patients, obtaining prior authorizations, chart verification prior to patient appointments and obtaining any pertinent medical information/documentation from outside providers or previous physicians.
- Compiles and assists provider with the follow up of lab/imaging results and reports to patients as directed in a timely manner.
- Responsible for organization, cleaning and re-stocking of exam rooms.
- Demonstrates appropriate use and disposal of hazardous waste.
- Fields patient phone calls, electronic messages and triages, prioritizes messages and responds accordingly.
- Assists with requests/obtains patient medical records.
- Demonstrates proper infection control procedures and follows universal precautions.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the possibility of medical errors.
- Other duties as assigned.
EDUCATION & EXPERIENCE:
- Current NYS LPN license required.
- Experience as an LPN in a physician's office, preferably in Internal Medicine.
- Current Infection Control Certificate is required.
- Basic Life Support (BLS) certification is required.
QUALIFICATIONS & REQUIREMENTS
- Ability to assess, plan, implement and evaluate nursing care according to individual needs.
- Must demonstrate an understanding of clinical risk.
- Familiarity with EMR systems preferred.
- Flexibility to work one Saturday per month
- Travel to any of the other Medical Group locations may be necessary upon request.
- 4 Days a week 7:00AM-5:30PM (day off during the week varies)
