Education: Graduate of an accrediting Licensed Practical Nursing Program.
LicensesCurrent New York State Licensed Practical Nurse license.
CertificatesValid basic cardiopulmonary resuscitation certification (CPR)

Job Summary:
- Reviews Medical Record for vaccination administration after receiving order from provider. Addresses any concerns with ordering provider.
- Obtains correct vaccination(s) from refrigerator. Compares and confirms vaccination(s) and dose ordered to what was retrieved
- Correctly identifies patient in exam room using name and date of birth
- Administers vaccine(s) as ordered
- Disposes of medical waste and hazardous waste, as per OSHA guidelines and clinic protocols
- Documents vaccine type, manufacturer, lot number, and anatomical location of administration in the EHR
- Provides patient and/or guardian with most current Vaccination Information Sheet (VIS)
- Updates new VIS when notified by CDC and shares with Satellite site MAs
- Reviews CIR, generates list of needed vaccines and provides completed list to Front Desk/Registration Staff for recalls.
- Updates and revises CIR of any patients' information and scans vaccine records into eCW
- Monitors and records twice daily refrigerator and freezer temperatures, as well as minimum and maximum refrigerator and freezer temperatures, resetting the memory each time
- Assists MAs at Satellite sites with any questions/concerns as well as performs satellite-site visits for vaccine/refrigerator compliance when assigned.
- Prepares Excursion Incident reports in conjunction with all Shelter site MA's or other LPNs; calling manufacturers and sending completed reports to Director HIM & QI for review before she shares with DOH.
- Prepares a Clinical Inventory Sheet (CIS) of the monthly inventory of all vaccinations at assigned site
- At site, stock medication cupboard will be checked monthly for any expired medications; rotating oldest medications to front; discarding those already expired
- Reports VFC expired or spoiled vaccines electronically in the CIR for each specific site, and coordinates packaging of spoiled vaccines for pickup by the delivering entity
- Enters newly received vaccine information into eCW; i.e. vaccine name, manufacturer, lot number, expiration date, quantity.
- Follows and demonstrates strict infection control guidelines to ensure proper clinic environment
- Follows sharps policy and procedure and demonstrates understanding
- Participates with clinical staff for in-service training, to ensure that nursing staff functions meet quality improvement standards.
