Must work well independently and in combination with a multidisciplinary team to provide efficient and effective treatment. Assesses needs and works directly with the patient on completion of treatment plan goals. Provides counseling, education, and support to patients and their families in individual and group sessions. Must have excellent written and verbal communication skills for documentation and therapeutic interaction purposes. Periodically provides in-service training to staff on unit. Interacts with Physicians and community agencies to provide continuity of care for the prevention of recurring problems and case management discharge planning. Serves as a representative of the Behavioral Health Center by presenting community education programs. This requires excellent organizational and presentation skills as well as extensive knowledge of mental health.GENERAL REQUIREMENTS•Adheres to the hospital and departmental attendance and punctuality guidelines•Performs all job responsibilities in alignment with the core values, mission and vision of the organization•Performs other duties as required and completes all job functions as per departmental policies and procedures•Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs)•Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.•Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs.
