Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)Location: Montour Falls, NY. Shift Time: 3:00 PM to 11:00 PM.Shift: EveningShifts per week: 5Responsibilities:
- Administer medications, following physician's orders and nursing protocols.
- Monitor and report any changes in residents' conditions to RNs or physicians.
- Provide direct patient care including wound care, IV management, and assistance with activities of daily living.
- Ensure accurate charting and documentation in the electronic health record, including using Point Click Care software.
- Assist in admissions, transfers, and discharges.
- Collaborate with the healthcare team to ensure the best possible outcomes for patients.
- Participate in care planning and assessments.
- Adhere to infection control standards and maintain a clean and safe environment.
- Assist with residents' personal care as needed.
Qualifications:License:
- Active LVN or LPN license in the state of New York.
Experience:
- Prefer previous experience in Skilled Nursing Facility (SNF) or Long-Term Care (LTC).
Certifications:
- Current Basic Life Support (BLS) certification.
