Job Summary and Qualifications

The Licensed Practical Nurse/Licensed Vocational Nurse (LVN/LPN) delivers high quality, patient-centered care by performing a variety of delegated basic patient care services. In collaboration with the RN and other members of the care team, the LPN/LVN provides individualized, comprehensive care consistent with the scope and standards of the specialty. Perform high quality direct patient care within the scope of the licensed Vocational Nurse Practice Act, with emphasis on the out-patient in the Transplant Center

What you will do in this role:

• Care for adult patients in the Solid Organ Transplant Clinic under supervision of RN
• Patients teaching: medication, procedures, follow up and other identified needs
• Physician clinic support
• Patient scheduling
• Gather, enter and maintain data/documentation related to Transplant Clinic patients consistent with established quality and integrity standards
• Participate in Quality Improvement
• Point of Care Testing
• Follows through on patients' needs through coordination of services
• Supports the medical record integrity

What qualifications you will need:

• Graduate of an accredited school of vocational nursing
• Adult Clinic care experience at least 2 years
• Two years of acute care clinical experience within the last 5 years preferred
• Current license registration with the Texas State Board of Vocational Nursing
• Current American Heart Association or American Red Cross BLS provider
• The LVN receives additional training and education through general orientation, department orientation, in-service and continuing education
