A few details about the role:

• Works closely with the Lifespace Community leaders to promote and assess the needs of residents in the community who need support or have wellness changes.
• Participate in community meetings to represent and promote Lifespace Personal Services
• Ensure resident satisfaction levels are achieved through quality and service delivery excellence.
• Review resident referral information and respond to Lifespace Personal Services requests and inquiries.
• Promote and market personal services and wellness programs by conducting resident clinics, events, and sponsoring resident education.
• Evaluate as needed the physical, emotional, and social needs of residents.
• Assist in monitoring resident for changes in condition, medication & nutritional needs and update care plan, physician, and family as necessary.
• Participate in the weekly Transition of Care or Risk Meetings within the community.

And here's what you need to apply:

• 3 years' experience leading and managing a personal services agency or healthcare team.
• Registered Nurse license is required.
• Bachelor's or master's program in nursing, public health, or administrative/management studies is preferred.
