Case Manager, LMSW - sign on bonus eligible, up to $4kArms Acres is looking for a Clinical Case Manager to join our inpatient team in Carmel, NY. In this role, you will provide comprehensive clinical case management services that includes assessments, individual, group and family treatment, treatment plan coordination, referent relations, discharge planning, and intervention with client support systems, including social service programs and managed care.
