A healthcare system in the Bronx is actively seeking a compassionate and patient-oriented Behavioral Health Clinician (LMSW / LCSW / LMHC / LMFT) to join their staff. In this role, the Behavioral Health Clinician (LMSW / LCSW / LMHC / LMFT) promotes a safe, cooperative and professional healthcare environment to ensure optimum patient care. Responsibilities The Behavioral Health Clinician (LMSW / LCSW / LMHC / LMFT):
- Gathers, analyzes, and evaluates data collected relating to the patient's bio-psychosocial and family problems, current mental status, and individual/family functioning
- Completes Intake Assessments, as requested by Clinical Supervisor
- Completes diagnostic evaluations and develops treatment plans, acting collaboratively with the patient and the treatment team in order to define appropriate goals, outcomes and time frames for treatment ability DSM-V
- Follows up with patients who fail to appear for appointments and documents the results of that follow up
- Maintains networking and referral contacts with other mental health /social service providers
- Interacts with other staff members of the Center and with community agencies in a professional manner
Qualifications
- Active New York State LMSW, LCSW, LMHC, or LMFT
- Working knowledge of current word processing software
- Ability to maintain accurate records and to interpret information
- Ability to integrate psychological theory, research, and practice
- Excellent communication skills
- Ability to organize and prioritize multiple tasks effectively
- Willingness to work cooperatively to support and achieve Department/Program and Center goals and objectives
- Ability to accept responsibility for tasks assigned
- Ability to work professionally and cooperatively with a multidisciplinary team approach
- Demonstrated cultural competence and ability to work as a team member
- Demonstrated ability to work with a multi-disciplinary team approach demonstrated ability to coordinate managed care activities and interface with other managed care providers
- Ability to organize and prioritize multiple tasks effectively and use discretionary time effectively
- Ability to counsel and provide support to patients and/or collaterals
