A health services organization in New York City is currently seeking a new Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW) to join their Assertive Community Treatment (ACT) team at one of their inpatient mental health facilities in Manhattan. Responsibilities: The Social Worker (LMSW / LCSW) will be responsible for: Providing the expected number of services to clients each month, majority of which are in the community or patient's homes Completing required documentation in a timely and high quality manner Working collaboratively with clients, their families, and community partners to assist clients in achieving their treatment and recovery goals Coordinating client care through close involvement with hospitals, substance use treatment facilities, etc. Screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations Participating in the development of treatment plans Counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning Providing client/patient advocacy and crisis intervention Developing, coordinating and evaluating services and support systems Qualifications: 1+ year of relevant work experience Active NYS Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW)Training and/or experience in integrated Mental Health and Substance Abuse assessment & treatment, Motivational Interviewing techniques, and Stage-wise Treatment; or,Training and/or experience in Family Assessment & Intervention, Psychosocial-education and/or other Family Support services, including Cognitive Behavioral strategies
