Registered Nurse - Home Care Lancaster PA, 17601 ( 13 weeks ) - 5 X 8s • 40 hrs. / Week 8 hrs. / Day - AM Description: Days 8-5 (1 hr. for lunch) flexibility available. Every 4th weekend, Employee must use their own vehicle. Job Summary We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Care team. In this role, you will deliver high-quality, personalized care to patients in their homes. You will work closely with patients, families, and interdisciplinary teams to promote recovery, health, and independence in a home setting. Key Responsibilities Patient Care: Provide comprehensive nursing care to patients in their homes, including assessments, wound care, medication administration, and other treatments as prescribed. Care Planning: Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and the healthcare team. Monitoring: Regularly monitor patients' health status, noting changes and responding promptly to complications or emergencies. Patient Education: Educate patients and families on managing conditions, medication use, and preventative health practices to enhance independence. Documentation: Maintain accurate, detailed, and timely documentation of care in electronic health records (EHR) in compliance with organizational and regulatory standards. Coordination: Collaborate with physicians, therapists, social workers, and other team members to ensure holistic and coordinated care. Regulatory Compliance: Adhere to all policies, procedures, and standards, including infection control and patient safety protocols. Education: Graduate of an accredited nursing program (Associate or Bachelor of Science in Nursing preferred). Licensure: Current and unrestricted RN license in the state of practice. Certifications: Basic Life Support (BLS) required. Certification in Home Health Nursing (CHPN) is a plus. Experience: Previous home care or community nursing experience preferred. Skills & Competencies Excellent assessment and critical-thinking skills. Strong communication and interpersonal abilities to work with diverse patients and families. Time management and organizational skills to manage multiple patients efficiently. Proficiency in using technology, including electronic health records and mobile devices.
