Job Description & RequirementsJob OverviewWe are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The Star+Plus (LTSS) Service Coordinator Level 1 is responsible for overall management of member's case within the scope of licensure; provides supervision and direction to non-RN clinicians participating in the member's case in accordance with applicable state law and contract; develops, monitors, evaluates, and revises the member's care plan to meet the member's needs, with the goal of optimizing member health care across the care continuum.StarPlus Service Coordinator Level 1 Licensed RNLocation: 77081- Harris County (Local Only) can applyMust reside in Harris County, TexasMust be willing to travel in the service areaNO RTO in the 1st month of trainingLocal Travel rates40hours per week (within 30 miles from the facility)$47.50/hour$71.25/hour - Overtime Rate$1,900 Weekly GrossFor RN position - candidate needs to be within 30 miles of the five cities below and must provide a Driver's License in the submittal profile to show proof of residency in these areas:o Clear Lakeo Galvestono Seabrooko Baytowno Channelview· Required to go into the office every day from 8am-5pm CT for the 1st month for training (location is 4888 Loop Central Dr. Suite 600 Houston, TX 77081)· Must have Managed Care Organization experience· Bachelor's or Master's degree based on required experience in health, psychology, sociology, social work, nursing, or another relevant human services field.· 3-5 years nursing experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar roleJob Types: Full-time, Contract, TemporaryPay: $1,900.00 per week
