Job Description & RequirementsOverview:Occupational Therapy works closely with PACE Physician, rehabilitation staff andother interdisciplinary team (IDT) members to maximize participant independence and safetyas well as enhance performance of ADLs. The Occupational Therapist collaborates with thePhysical Therapist to efficiently operate WelbeHealth rehabilitation department and is amember of the WelbeHealth management team and facilitates collaborative relationshipsacross WelbeHealth and with external stakeholders to promote the WelbeHealth values, teamculture and mission.Conduct Occupational Therapy assessments to determine the Occupational Therapycare needs, preferences and goals of the participants and actively participate in IDTmeetings to develop participant care plans.Deliver and document Occupational Therapy interventions in accordance with physicianorders and/or as agreed upon in the participants' care plans including but not limited tothe use of devices, techniques and activities to promote muscle re-education,perceptual-motor and cognitive function, balance, endurance and other factors affectingthe level of independence in performing ADLs.Supervises the Therapy Assistants including providing training, instruction and guidanceneeded to implement Functional Maintenance Programs (FMP).Provide training and instructions to staff, participants and caregivers/families in areas ofOccupational Therapy in which they may work with participants.Order, track and manage maintenance for equipment.Remain alert to pertinent input from other team members, participants, and caregiversand update the IDT promptly of any changes in participants' condition or functionalstatus.Work with the other members of the IDT to support smooth care transitions betweensettings (e.g., hospitals, skilled nursing facilities, home, etc.).Participate in end-of-life care coordination and support.Manage direct reports including training, supervising and mentoring.Skills & Experience/Education: Minimum of two years Occupational Therapy experience in a clinical setting with a frailor elderly population.Bachelor's Degree in Occupational Therapy from Accredited Program (AOTA and ACOTE standards).Initial Registration from National Board for Certification in Occupational Therapy.Licensed and/or eligible for Licensure as required in the State of California.BLS – American Heart Association.Job ID #15451342
