- Weekend and weekday call rotation. Weekends are apprx. every 4th/ 5th. Weekday call approx. 1/week

- Provides individualized nursing care/assessments at the home/residence of the client diagnosed with acute, chronic and complex conditions.
Roles & Responsibilities
- Analyzes and differentiates clinical data to appropriately diagnose nursing problems; and identify nursing diagnosis.
- Provides individualized care to clients with complex care needs including pediatrics, geriatrics, and mental health patients.
- Demonstrates the ability to utilize critical thinking skills in the home environment and identify situations requiring physician notification or emergent care.
- Demonstrates the ability to care for clients with a variety of diagnosis' including but not limited to orthopedic, surgical, medical, cardiac, wound/ostomy, and home infusion needs.
- Provides health coaching/education to patients/families and populations focused on achieving lower costs, engaged patients and reduced hospitalizations and ER/Urgent visit usage.
- Delivers nursing care for the clients in their homes, utilizing appropriate available resources.
- Involves the client/caregiver in goal development and plan of care development.
- Involves the client/caregiver in developing and carrying out the plan of care.
- Educates client/caregiver on disease process, treatment plan, and methods to increase independence for discharge planning. Evaluates and documents client's physical and emotional reactions to treatments, care, and medications in the electronic medical record (EMR).
- Conferences with other disciplines appropriately to provide an interdisciplinary approach to meet all identified client care needs.
- Assesses existing and potential problem areas, collects data, and reports significant information to the Clinical Manager or Clinical Leader.
- Functions purposefully in and adjusts to emergency situations as they arise.
- Instructs and supervises the home health aide, LPN, or other services provided to the client in their home requiring an RN supervision.
- Submits and maintains accurate, current, and complete documentation according to policy on a timely basis.
- Performs other responsibilities as requested by leadership. .
- Maintains required department confidentiality and abides by HIPPA regulations
- Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.
- Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development and participates in medical center and/or departmental education programs.
- Communicates with and performs duties following the philosophy of reflecting the organization's Mission, Values, and cultural beliefs.
- Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment.
- Performs other responsibilities as requested by supervisor, manager or department head.
QUALIFICATIONS
- BLS and ACLS
- National Institutes of Health (NIH)
- Sound knowledge of anatomy, physiology, pathology, and medical terminology.
- The ability to stand for extended periods.
- Strong technical skills.
- Knowledge of medical terminology
- Passionate about patient care
- Requires a minimum of two years of current RN Home Health experience.
- Graduate from a fully accredited RN program
- Active unencumbered Iowa RN state license
- One year of Cerner experience
