Requirements –

• Board Certified/Board Eligible - REQUIRED
• Active MA License - Preferred
• Flexible on availability
• Covid and flu vaccination - REQUIRED
• BLS - REQUIRED
• DEA - Verbal Confirmation Required

• Schedule (Shifts and Hours): 8a-12p, 1-5p

• Describe Office Ancillary Staff (e.g., Number of RN's, LPN's MA's etc.): 2 MAs plus LPN support derm. Additional staff in office to cover

• Required Surgical Procedures: Biopsy, Excision, Electrodesiccation, Dermabrasion
• Estimated Credentialing Timeframe: 1 week after all documents submitted
