Requirements –

• Board Certified - Required
• Active IL license - Required
• Provider that speaks Spanish - Highly preferred, please note
• DEA - Verbal Confirmation Required
• Clean background/malpractice - Required

Schedule –

• Outpatient Coverage: 9am – 4pm. Clinic. Must be available full time - no weekends

Duties

• 30% Hematology70% OncologyNumber of Physicians in Practice: 1 remaining physicianHem/Onc only practiceAdults only No Radiation Oncology on-site, but connection to Rad Onc team at Loyola University Medical Campus, which is 5 miles from practice.No APP's in practiceDocumentation System/EMR: epicEstimated Credentialing Timeframe: 3 months Practice Setting: Outpatient Free-Standing Community Cancer Center Average Number of Patients Per Day: 16-20 Trauma Level/Designation (Hospital): Level 2
