(Request full hospital detail, if interested) REQUIRED: · WA state license; active, unrestricted · Board Certified Obstetrics & Gynecology; Board Eligible (2-5 years post training). · DEA NC/PRES with: · Delivery volume over the last 2 years. · NPDB self-inquiry report: The NPDB - Home Page (hrsa.gov) + explanations (if applicable). · Updated CC (with MM/YY; all gaps over 30-days explained) · 14 shifts per month of availability (weekdays + weekends).
