+ Compliance with MAT (Medication-Assisted Treatment) program protocols (training is provided)
Under the supervision of a Registered Nurse, the LicensedPractical/Vocational Nurse Clinic Site works in a remote clinic location toserve a specific population's nursing needs. The nurse demonstrates ability tomake clinical judgments in an effective and efficient manner with supervision.The nurse demonstrates critical thinking and performance ability in the coordinationof patient care. Consistently performs according to nursing standards and isaccountable in providing safe, quality patient care.
Essential Duties and Responsibilities:
+ Under the direct supervision of a Registered Nurse utilizesthe nursing process to assess, plan, implement and evaluate patient care
+ Identifies and prioritizes actions based upon patient carerequirements
+ Performs interventions according to identified priorities,plan of care, and the Clinic policies and patient care outcome standard
+ Provides a safe, comfortable and therapeutic patientenvironment
+ Initiates and evaluates patient and/or family education
+ Documents patients response to nursing and otherinterventions
+ Monitors the safety and effectiveness of the environment andequipment
+ Participates in activities to facilitate good interpersonalcommunications with the chosen facility
+ Administers vaccines or any necessary medications
+ Makes follow-up phone calls to the patients
+ Inputs information into the computer
+ Helps stock and maintain the cleanliness of the facility
Minimum Requirements:
+ Current Licensed Practical/Vocational Nurse License
+ One year experience as an LPN/LVN preferred
+ One year experience in a clinic setting preferred
+ Complies with all relevant professional standards ofpractice
+ Participation and completion of Amergis' Competency programwhen applicable
+ Current CPR if applicable
+ TB Questionnaire, PPD or chest x-ray if applicable
+ Current Health certificate (per contract or stateregulation)
+ Must meet all federal, state and local requirements
+ Successful completion of new hire training as applicable tojob site
+ Understand patient confidentiality and HIPAArequirements
+ Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
+ Computer proficiency required
+ Must be at least 18 years of age
