Job Description:

AOM Healthcare is a premier long term care company operating twenty one long term care facilities throughout the state of Ohio and is now hiring Full-Time and Part-time (LPN) Licensed Practical Nurses at Concord Care of Hartford, located at 3090 5 Points Hartford Rd, Fowler, Ohio.

Our team consists of experienced, energetic, dedicated, and compassionate RNs, LPNs, and STNAs.

We are currently seeking skilled, compassionate, dedicated, and driven Licensed Practical Nurses (LPN) to welcome our ever-growing team!

Job Duties and Requirements:

  • Valid Ohio State License(s)
  • Reliable Transportation
  • Current license as a Licensed Practical Nurse in the State of Ohio required.
  • Is a graduate of an accredited school of Licensed Practical Nursing.
  • Successfully completes facility conducted orientation, mandatory training and in-service programs.
  • Previous long term care experience is required. Supervisory experience and organizational skills in a long term care setting preferred.
  • Must meet health assessment requirements, including two stage Mantoux skin test.
  • Is listed in good standing as confirmed on the Ohio State Department of Professional Credentialing.
  • Possesses excellent technical assessment and documentation skills, and leadership quantities.
  • Must be capable of performing the essential functions of the job, with or without reasonable accommodations.
  • Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required
