The LPN demonstrates excellent clinical and communication skills and exhibits patience and respect for all patients, families, and team members.Essential Job Responsibilities of an LPN:
- Provides skilled nursing care based on a compassionate and holistic approach.
- Follows a comprehensive care plan that addresses physical, emotional, cultural, and spiritual needs of the patient and family with input from the patient, family, and interdisciplinary team.
- Advocates for pain/symptom control.
- Provides telephone triage support to Clinical Team.
- Teaches, supervises, and counsels the patient and family regarding care concerns.
- Maintains close communication and involvement with the RN Case Manager, attending physician, home health agency, spiritual advisors, and other persons that may be participating in the care.
- Collects and reports patient data to RN as requested.
- Provides nursing care and supports Hospice Home staff when needed.
- Functions as Hospice Aide when needed.
- Participates in ongoing 24-hour call with RN backup.
- Attends and participates in IDT meetings as needed.
- Meets safety needs of patients and uses equipment safely and appropriately.
- Documents and records care provided according to agency standards.
Qualifications of an LPN:Required Knowledge, Skills, and Abilities of an LPN:
- Familiarity with community health organizations, practices, and resources.
- Familiarity with the Hospice philosophy of care for the dying and in agreement with the principles of HOL care and a commitment to it.
- Ability to make sound clinical judgements and report them in a timely manner.
- General technical abilities with nursing functions, such as catheter insertion, dressing changes, equipment function and maintenance.
- Ability to function in a home setting with RN supervision.
- Demonstrates excellent customer service, both internally and externally.
- Possesses strong communication skills and the ability to interact positively and professionally with patients, families, and other team members.
- Demonstrates interest, compassion, and empathy for the terminally ill and is comfortable with issues of death and dying.
Education/Credentials of an LPN:
- Graduate of approved School of Nursing.
- Current Michigan LPN License.
- CPR certification, required.
Experience of an LPN:
- Two years nursing experience in hospice, home health, or oncology nursing, preferred.
- Experience with dying patients and their families, hospice experience preferred.
- Experience in health care, required.
Additional Requirements:
- Valid driver license and automobile with current insurance.
