Licensed Practical Home Care NurseFT/PT 1st and 2nd shiftTitle: Licensed Practical Visit NurseFacility: Fidelity Health CareJob Reports to: Clinical Nursing SupervisorAs a Licensed Practical Nurse (LPN) you are responsible for the delivery of home care services for patients. In the LPN role, you will provide care within their scope of practice as ordered by the physician and follow policies and procedures while regularly reporting to the nursing supervisor.
