Role and ResponsibilitiesAdminister practical nursing care to patients in accordance with physician's orders under the direction and supervision of a Registered Nurse.Essential FunctionsUnder the direct supervision of an RN Case Manager, and within the scope of practice for a Licensed Practical Nurse:Provide health education according to the teaching plan established by the RN.Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.Observe signs and symptoms, and report patient's reactions to treatments including drugs and changes in the patient's physical or emotional condition, to the physician and RN Case Manager.Document any services rendered to the patient, changes noted in the patient's condition, and/or family and home situations.Document communication with RN Case Manager and physician.Other duties as assigned.This job description reflects management's assignment of essential functions; and nothing in this herein restricts management's right to assign or reassign duties and responsibilities to this job at any time.Education RequirementsAssociate's degree required.Diploma as a Licensed Practical Nurse from an accredited program.Qualifications and RequirementsCurrent, valid LPN license issued from NYS Department of Education.Experience in home health care preferred.Community health experience preferred.Work EnvironmentThe LPN is primarily in a non-office setting and may be exposed to outdoor conditions.The working conditions are classified as medium work:Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
