Overview
Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.
Team 1 Covers The Northern Nassau Area; Bayville, Bethpage, Carle Place, Great Neck, Hicksville, Kings Point, Lake Success, Manhasset, Mutton town, Old Bethpage, Port Washington,
Roslyn and Westbury.
* 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details
Our Home Care LPN's:
* Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
* Measure vital signs, Administer IVs, change dressings, clean wounds as needed
* Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
* Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
* Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment.
* Minimum Qualifications:
* New York State Licensed Practical Nurse (LPN) license
* Minimum of one year of recent hands on experience
* Basic Life Support Certification
Salary Range
USD $32.00 - USD $40.00 /Hr.
