The LPN works under the supervision of a Registered Nurse. The LPN will provide nursing supports to individuals as required, including assisting with direct care of Individuals when necessary. Review staff documentation related to weights, seizures, bowel management, equipment, glucometer readings, vital signs, etc., and taking appropriate action if indicated.Responsibilities
  • Complete nursing notes as needed including significant medical changes, interventions, recommendations, and communications with providers, day services and families.
  • Review and maintain appropriate medical records in accordance with OPWDD requirements and best practice standards
  • Monitor individual's medical needs and health status on a continuous basis, provide treatments and care, document medical histories, implement appropriate protective and preventive measures regarding communicable diseases and maintain appropriate medical records.
  • Ensure that individual's prescriptions are filled/refilled, record physician's medication orders, monitor individual's medication administration sheets to ensure that medications are being administered properly, report medication errors appropriately, and disposing of outdated or discontinued drugs according to policy.
  • Review of clinical data and upload medical information in My Health
  • Update My Health Information in Evero
  • Add Health Care Providers
  • Complete & Provide weekly report to IDT team
Qualifications
  • Associate degree in Nursing and two years of professional experience in delivering of health care to people with Intellectual Developmental Disabilities.
  • Licensed as a Licensed Practical Nurse required.
Abilities:
  • Excellent presentation and writing skills.
  • Ability to translate agency philosophy into practice and implementable techniques and interventions and be able to represent the agency professionally.
  • Knowledgeable in Electronic Health Records
  • Valid Driver's License for travel to multiple locations.
